	Diabetes PCC Program Download Now This program was originally written to allow practices to easily obtain the information needed for reporting for the Primary Care Collaborative. The information required has different timescales and slightly different targets to the New GMS Contract. It was designed to allow practices to e-mail the results and to allow easy printing and saving of results for the practices own use. Once in use it was expanded to allow practices to look at lists of patients who were out with the targets or did not have recordings of some of the parameters i.e. Blood Pressure, HBA1c and Cholesterol. Finally it has been developed to look more closely at some of the New GMS Contract Parameters including Missing Indicators and Exclusions. The latest version is 3.1.0.0   .Running the queries will produce the screen as shown below with the numbers as required for the collaborative         Once all the information has been organised it is possible to look at other information with regard to recording of HBA1C, Cholesterol and Blood Pressure           Once generated the reports cab be saved or printed and the overall numbers for each seen and graphed.
	There is also a section based on the new GMS contract rules which will allow viewing of results, Missing Indicators and Exclusions         Version 3.1.0.0 changes Program updated to reflect changes in the GMS contract for 2006-2007
